Introduction to the JED Campus Assessment

JED Campus Baseline Assessment

Thank you for participating in JED Campus. While completing this self-assessment, please keep in mind the following:

• It is our recommendation that you read through the entire assessment to identify the teams/departments with whom you will need to collaborate in order to complete the assessment with the most comprehensive information.
• This assessment should be completed by your JED Campus Team. There are different approaches to completing the assessment. Some JED Campus teams have one designated leader who gathers the information from team members and completes the assessment. Other JED Campus teams break into subgroups to fill out sections that are relevant to them (relevant stakeholders have been noted at the beginning of each assessment section). And others work together as a full JED Campus team in person to fill out the assessment. Work with your team to identify which method is the best for your campus.
• This is a forced choice instrument and you must choose from the options provided. If you need to explain your choice or provide additional information, please do so in the open-ended question at the end of each section.
• Please answer every question. Some questions have sub-questions that only need to be answered dependent upon your answer to the initial question. Please follow the instructions carefully.
• In a section where it prompts you to describe in your own words any additional programming or activities you may offer, please be sure to not duplicate information that will be asked in later questions or assessment sections.

JED Campus Follow-up Assessment

Congratulations on reaching year 3 of your JED Campus journey. We deeply appreciate your school’s commitment to promoting mental health, preventing suicide and limiting substance use. While completing this self-assessment, please keep in mind the following:

• The main goal of this assessment is to capture all the changes and transformations your school has implemented over the last 3 years as a result of your participation in the JED Campus Program.
• This assessment should be completed by your JED Campus Team. It is our recommendation that you read through the entire assessment to identify the teams/departments with whom you will need to collaborate in order to complete the assessment with the most comprehensive information.
• You might notice that this assessment is very similar to the one you filled out when you entered into the JED Campus program. We ask you not to review/check the responses you selected then, since that would negatively impact the validity of your answers.
